Upper Cumberland Development District has an opening for a Relative Caregiver Program (RCP) Case Manager. The RCP Case Manager will effectively determine the needs of relative caregivers and match those needs with appropriate services.

Duties include:

  • Provide case management services, support, and referrals to resources to caregivers enrolled in the program
  • Schedule monthly home visit appointments to meet with caregiver and eligible relative children (flexibility required: scheduling will include evening and possibly weekend home visits to accommodate families)
  • Complete initial and yearly required eligibility paperwork to maintain caregiver’s eligibility for the monthly RCP stipend payment. Information is maintained per the RCP Program Guide and agency protocol
  • Be an advocate for the best interests of the caregiver and children. Case managers may provide supportive engagement with families. Case managers should strive to provide short-term, targeted support which focuses on stability, problem-solving, and connection to resources
  • Home Visits:
    • Gather information about the Family Dynamics (why was the child placed in the home, birth parents’ involvement, children’s activities, disabilities, etc.)
    • Discuss family needs and appropriate referrals to resources that can benefit family
    • Obtain documents required for program eligibility such as: Driver’s License (all adults in home); Social Security Cards; Birth Certificates for all eligible children; Court papers; Proof of Relationship (Court papers, birth certificates, tax returns, medical records, or as listed in the RCP Program Guide); review as appropriate – income verification of all income coming into the home to assist in planning
    • Review, fill out, and discuss all paperwork with the caregiver, obtaining signatures where needed: refer to the agency protocol and RCP Program Guide
  • Case Management and Office Duties:
    • Manage ongoing Case Management responsibilities for assigned families.
    • Ensure caregivers are added to inner-office call and mail lists, data sheets for reporting, and adding clients into the current electronic case files required by DCS (ie. TFACTS/CFIRST!)
    • Case management service notes are entered timely into the electronic recordkeeping system (within 14 days)
    • Maintain a physical client file containing all documents and forms in the correct sections.
    • Develop a system of tracking caregivers and home visits monthly, to ensure face-to-face visits occur with each eligible family and child.
    • Any time there is a change in the dynamics of the home, update coworkers, in-office data keeping, and Kinship Specialists as appropriate.
    • Bring requests to financial meetings for review and approval by the financial oversight committee. If approved, make arrangements to get the assistance to the family (ie paying bills, shopping for material items, etc). Follow agency procedures for purchasing items, completing Purchase Requests, requesting an invoice, collecting a W-9 and submitting to the Program Director for approval.
    • All RCP providers and staff are mandated reporters under Tennessee law. Any person who has reasonable cause to believe that a child has been abused or neglected must immediately report the concern, regardless of the individual’s role, relationship to the child, or involvement in the RCP program.
    • All information obtained through the RCP program is handled in accordance with applicable confidentiality laws and policies. Providers and case managers must protect the privacy of children and families while supporting coordinated service delivery.

Qualifications:

  • Bachelor’s degree in a human service field (preferred)
