Upper Cumberland Development District/Area Agency on Aging and Disability has an opening for a CHOICES Home and Community Based Services (HCBS) Qualified Assessor.  The CHOICES HCBS Qualified Assessor will complete in-home assessments including, but not limited to, Pre-Admission Evaluations (PAE) and Medicaid financial applications; obtain financial and medical documentation necessary for eligibility determination; coordinate with physicians’ offices to obtain medical documentation; determine needs, community resources and coordinate services with agencies/groups for older adults and individuals with disabilities.

Duties include:

  • Complete in-home assessments including, but not limited to:
    • Pre-Admission Evaluations (PAE) and Medicaid financial applications
    • Obtain financial and medical documentation necessary for eligibility determination
    • Coordinate with physicians’ offices to obtain medical documentation
  • Determine needs, community resources, and coordinate services with agencies/groups for older adults and individuals with disabilities
  • Assist with CHOICES outreach/marketing activities and completing annual surveys for CHOICES members

Qualifications:

  • Licensed Practical Nurse, Registered Nurse, Licensed Social Worker, Physician Assistant, Nurse Practitioner, Licensed Physician, or a Bachelor’s Degree in social work, nursing, education or other human service (e.g. psychology or sociology) with prior approval by TennCare
  • Proficient computer skills
  • Written and verbal communication skills
  • Ability to work independently and as a team
  • Upon hire, complete TennCare’s online training and pass the HCBS Qualified Assessor exam within the timeframe required by the Bureau of TennCare
