The Upper Cumberland Development District (UCDD)/Area Agency on Aging and Disability (AAAD) has an opening for an Information and Assistance (I&A) Specialist/Options Counselor.  The I&A Specialist will determine the supportive needs of family caregivers, individuals with a disability, and older adults; conduct comprehensive phone screenings for individuals applying for Home and Community Based Services (HCBS); and provide clients with resource information and/or refer them for other services that can help meet their needs.

The Options Counselor assists individuals in making informed decisions about long-term care supports with a focus on HCBS. The Options Counselor completes in-home assessments to assess needs and determine eligibility for HCBS programs, develops and manages care plans, coordinates HCBS services with subcontracted providers, and connects individuals with other community resources. The caseload for this position includes clients residing in the following counties:  Cannon, Warren, White, and Van Buren.

Requirements include:

  • Bachelor’s Degree in social work, psychology, gerontology, sociology, counseling, nursing, or equivalent degree; or a minimum of completion of two (2) years of attendance at an accredited college or university and a minimum of 2 years of experience in the field of social work or a related field. Additional related experience may be substituted for education
  • Proficient computer skills
  • Excellent written and verbal communication skills
  • Ability to work independently and as a team
  • Ability to effectively multitask
